To file a claim, please review the instructions and fill out the form, both found below. All claims must be filed no later than 5:00 PM on Monday, June 17, 2024. Forms must be Notarized if submitted through the mail. If you choose to bring your form to our Independence office, and bring with you your valid California State I.D. the form will not need to be notarized.
